By submitting this form, I acknowledge that I have read and agree to follow all parade rules and regulations.

* Drivers must be at least 18 years of age and hold a valid driver’s license.

* All vehicles must be insured.

* No alcohol, drugs, or open flames are permitted.

* All entries must follow instructions from parade officials.

I, intending to be legally bound, hereby, for myself, my family, my successors, assignees, heirs, executors, administrators, forever waived, release and discharge any and all my rights, claims for damages, cause of action whether in law, equity or otherwise known or unknown, that I or any of them, may have against the All Things Tennessee Festival Committee, the Fentress County Chamber of Commerce, Fentress County Government, the City of Jamestown, all sponsors of the event, and their officers, directors, board members, employees, volunteers, independent contractors, agents and representatives, successors and assignees, for any injury, illness, or other harm suffered by me in or as a result of this event. I understand that there will be no refunds if the event cannot be staged or is cancelled for any reason. We reserve all rights to cancel this event and shall not be liable for any consequential damage. We also acknowledge that the committee(s) reserve the right to not approve applications to participate, and note that all participants must be a Fentress County based business, organization, non-profit, or resident. I attest that I understand precautions must be taken when riding in the parade in warm or cold conditions. And on the parade route, I will abide by the rules of the parade official. I also agree to abide by any decision of an appointed parade official or medical official relative to my ability to safely continue or complete the event. I further assume and will pay my own legal, medical, and emergency expenses in case of an accident, illness, or incapacity, regardless of whether I have authorized such expenses. I have read this waiver carefully and understand it fully. This signature certifies that I am responsible for my parade entry and have permission to participate in the All Things Tennessee Festival Parade. I have read the foregoing RELEASE AND WAIVER OF LIABILITY AGREEMENT, and by submitting this form I intentionally and voluntarily agree to its terms and conditions. Submission of this form further certifies that participants are in good physical condition and able to safely participate in the All Things Tennessee Festival Parade.
